Stonechurch Family Health Centre (Lead: David Price)

Stonechurch Family Health Centre is an academic teaching unit of the Department of Family Medicine at McMaster University. In addition to its blend of both academic and community physicians, Stonechurch also boasts a diverse interprofessional team, including nurse practitioners, social workers, chaplains, lactation consultants, and a variety of visiting specialists. Stonechurch Family Health Centre is committed to collaborative practice in the provision of quality primary care. This site is currently participating in a Primary Health Care Research Study (funded by Health Canada) to explore the development and effectiveness of occupational therapy and physiotherapy roles within the team.

McMaster Family Practice (Lead: Russ Springate)

McMaster Family Practice is a primary care teaching unit of the Department of Family Medicine, McMaster University. It is closely affiliated with Hamilton Health Sciences and has been located off the main campus site for the last two years. While its primary mandate is to provide a practice setting for family medicine residents, McMaster Family Practice prides itself on being a centre for multidisciplinary learning, and includes nurses, nurse practitioners, a chiropodist, and a social worker on its staff.

Regina Gardens (Lead: Anna Emili)

Regina Gardens is a Long-Term Care Facility located on the Hamilton Mountain. While they already show a commitment to interprofessional care, with a staff consisting of nurses, physicians, recreation staff, physiotherapists, social workers and dietary staff, Regina Gardens is excited for the opportunity to strengthen their collaborative care skills by participating in this project. They are an emerging educational training facility.
